Just realized I was ignoring the biggest red flag in my own contract.

A client kept asking for 'just one more small change' for free, and I saw they'd never signed the version with my revision limit. I was so focused on being helpful, I missed that they were avoiding the paperwork on purpose. Has anyone else had a client who was suddenly allergic to signing things?

Yeah my buddy had that happen. Client kept asking for "quick feedback" over email, then refused to sign the change order. Total nightmare.

Used to think email was fine for that stuff. Learned the hard way to get everything in writing now.

Totally feel that. After a client tried to walk back a whole list of email requests, my rule is now to send a summary doc after any call. Just a simple bullet list of what we agreed on, and I ask for a quick reply saying it looks right. It adds maybe five minutes to my process but has saved so many headaches. What do you do to confirm things?
