Man, that hits home. A buddy of mine had a winch fail because someone else swore they checked the pin, and @dylanwells is right, the crew was so busy blaming each other they almost missed a cracked weld. It's a tough balance for sure.

That "snapped lift bag line" thing reminds me of a time we nearly lost a whole dredge pump.

But honestly... sometimes you can check things too much. I've seen guys waste a whole hour second-guessing gear that was just fine, holding up the whole crew over nothing. That extra stress can make you miss the real problem right in front of you.
