The phishing test that backfired at our office in Dallas

Our IT guy sent a fake phishing email to everyone last month, and I clicked it. Got a 30 minute lecture about MFA, but turns out the test link bypassed our own spam filter. Has anyone else had a security drill that actually exposed a real flaw instead of just embarrassing people?

Kinda like when our smoke alarm went off from burnt toast... that filter thing is worse though.
