My boss pulled me into a meeting to 'check in' and then offered me a promotion.

It came out of nowhere because my last review was just okay. The new role means a 15k raise and I start managing two people next month. Has a surprise promotion ever actually worked out for anyone?

Wait, I think you might be reading that "just okay" review wrong. Sometimes a decent review is all they need if they see you handling extra work well. My friend got a promotion after a mid-tier review because her boss noticed she was the one fixing team problems quietly. The surprise part is stressful, but it often means they trust you more than the paperwork shows. Just go for it and ask for clear goals in the new role.

Man, that is such a good point. I've totally seen that happen where the official review is just average, but the person is actually the glue holding things together. Like the guy who always stays late to help new hires figure out the software, even though it's not in his job title. The bosses notice that stuff way more than a checkbox on a form.

A business book I read years ago talked about this. It said managers often have two lists: the official goals for reviews, and the real problems they need solved right now. If you're fixing those real problems, you become essential. Your boss likely sees you as someone who can handle more responsibility, even if your last review didn't spell that out. A surprise promotion can be a sign they value your practical skills over a perfect scorecard.
